School's Clinic / Ryan Health
Ryan Health runs a School-Based Health Clinic at the Park West Campus. The clinic is open and available to see students who attend HSHM, both in-person and remote.
Below you will find Spanish and English fillable consent forms that students should complete and have their parent or guardian sign. Once completed, they can email the form to school@ryanhealth.org or bring it to the school clinic.
Students are required to have these forms completed prior to their first visit. If a student has already completed the consent form in the past, they do not have to complete a new one. Please note that students who are 18 years of age or older can come to the clinic at any time and complete the adult consent form.
